    Preparing for Your Appointment

    We find it’s easiest to offer the most efficient office experience if you arrive at least 15 minutes before your scheduled appointment time. Before you leave home, make sure you bring with you the following:

    • a driver’s license or a valid, government-issued ID
    • a current insurance card
    • a list of all current medications you take
    • any paperwork your doctor sent you to complete

    Before your first meeting with one of our specialists, we encourage you to spend some time thinking about the goals you want treatment to help you achieve. Make a list of questions you have and concerns about treatment you’d like to discuss, and bring them with you. 

    In some situations, it can be helpful to invite a trusted family member or friend to accompany you to your appointment, as they may have a perspective that is relevant and valuable—and may be helpful to you in discussions after your appointment as you consider treatment options.

    MyChart Account

    We offer you access to MyChart, a free online tool that allows you to view personal health information, request appointments and send secure messages to your doctors.

    There are three ways to create a MyChart account:

    • Self sign-up. Visit MyPatientChart.org and click Sign Up Now. MyChart will verify your information, then you will be prompted to create a username and password. Self sign-up is only available for patients age 18 and older.
    • Email from your healthcare provider’s office. This email will include a link that automatically enrolls you in MyChart. Simply click the link and follow the instructions to activate your account.
    • Activation code from your provider’s office. If your healthcare provider gives you an activation code number, you can visit MyPatientChart.org and click Have An Activation Code? Enter the code number and follow the instructions to activate your account.

    People come to us for treatment from St. Louis and around the globe. If you and your family are travelling to St. Louis for care, you may qualify for assistance with lodging.
